Having your healthcare needs met by qualified professionals doesn't always mean a trip to the doctor’s office or the hospital. People recovering from an illness or an injury can, in many cases, have exceptional care delivered directly to their homes by the skilled and well-trained staff of YRMC Home Health.
Home Health care is a skilled medical service that allows health care professionals to assess, monitor and teach the patient and family in the comfort and familiar surroundings of the patient’s home. The goal of Home Health is to assist the patient in recuperating after an acute illness or work with patients with disabilities or chronic illness to regain an improved level of health and wellness.
Home Health specializes in helping patients achieve their highest possible level of independence with a plan of care designed to minimize the effects of an illness or a disability. Services are available 24 hours a day, seven days a week, and include:
- Nursing care: Registered nurses help with everything from wound care and IV therapy to managing medications. Home care aides are also available to assist with general and personal care.
- Physical therapy: A registered physical therapist uses exercise and special therapy techniques to help patients resume their normal activities.
- Occupational therapy: A certified occupational therapist helps patients learn to care for themselves, so they can remain as independent as possible.
- Speech therapy: A certified speech pathologist provides the services that can help patients overcome talking and swallowing problems.
- Medical social services: A social worker helps patients and their families with any social challenges related to a medical problem.
- Personal Care from a Homecare Aide
- I.V. Therapy
- Certified Wound Care Consultant
A Home Health RN covers phone calls and is available to Home Health patients 24 hours a day, seven days a week. Planned and scheduled visits are made between 8:00am and 4:30pm or as needed to fulfill the orders of your attending physician.
HOW DO I OBTAIN YRMC HOME HEALTH SERVICES?
Home Health services are available with a doctor’s referral. Patients currently in any hospital can ask their physician, nurse or discharge planner about obtaining Home Health Care from YRMC. Patients in their homes can contact their local primary care physician and ask for a referral to be called into the YRMC Home Health office. YRMC Home Health is state licensed and Medicare certified.
