CARE MANAGEMENT

YRMC Care Management – formerly known as Social Work Services – provides emotional support, counseling and guidance to YRMC patients and members of their families. Specialists in care management, who are highly trained and offer years of experience, are also available to help patients find practical solutions to a wide range of health-related problems.

Upon returning home from a hospital stay, many of our patients require additional care and support. Care Management strives to ensure continuity in care after our patients are discharged. Staff members assist patients and their families in coordinating medical, financial, legal and social services with available community resources.
